Sildenafil (Viagra®) is a phosphodiesterase-5 (PDE5) inhibitor used to treat erectile dysfunction (ED) in men. Although it has been proven to be effective for many men with ED, there are limitations regarding the use of Viagra as a treatment for ED in men. This study was conducted to explore the use of sildenafil as an additional treatment for ED in men with erectile dysfunction (ED) and to determine whether this medication was more effective than Viagra, which is a PDE5 inhibitor. A total of 573 patients with ED were included in this study. Demographics, age, sex, and duration of ED were the inclusion criteria. All patients underwent a PDE5 inhibitor treatment. Sildenafil was administered to the patients before treatment with Viagra. Patients were randomly assigned to receive either sildenafil (Viagra®) or placebo (placebo).

Patients were randomly assigned to receive either sildenafil (Viagra®) or placebo. Treatment with sildenafil was initiated at an initial dose of 25 mg, increased daily at a dose of 100 mg, and maintained for at least 4 weeks. Patients were encouraged to continue treatment for 4 weeks to determine their response and to have a baseline blood pressure and waist circumference measured. All patients were able to return for the follow-up visit at which time the patients were assessed for the presence or absence of ED. The incidence of adverse events was not different between sildenafil and placebo.

This study was deemed eligible if the patient's baseline blood pressure was within the following criteria: (1) normal or within 0.5 mmHg of systolic blood pressure (SBP) at baseline, (2) the patient's age was less than 18 years, (3) the patient's sex was not reported, and (4) the patient's duration of ED was at least 3 months. Exclusion criteria included: (1) the presence of known cardiovascular disease (e.g., hypertension, angina, or chronic thromboembolic disease), (2) previous history of adverse cardiovascular events (e.g., stroke, transient ischemic attack, or unstable angina), (3) the presence of any history of priapism (e.g., active or recent history of priapism), or (4) any medications that were used to treat ED.
The study population was primarily men who had sex with men (MSM) who were 65 years or older, were treated with Viagra®, or had a history of priapism. Men who had taken sildenafil and/or Viagra for more than 3 months, and had a baseline blood pressure less than 130/80 mmHg or more than 140/90 mmHg, were excluded from the study.
